Participants contribute data over time from daily life to help researchers understand real-world patterns.

Who is eligible?
Adults (18+) in the U.S. who have a cardiovascular disease, including but not limited to:
- Heart Failure (HF)
- Hypertension (HTN)
- Hypertrophic Cardiomyopathy (HCM)
- Dilated Cardiomyopathy (DCM)
- Aortic Stenosis (AS)

What will patients do?
Use the devices, answer in-app questionnaires, and provide access to their electronic health record for research purposes.
Most weeks' tasks take just 5-10 minutes to complete.
How long will patients participate?
The study will be open for up to 5 years.
The length of participation will depend on when the patient joins the study. They are free to withdraw at any time.
